Switching Swiss Health Insurer for 2027: Up to 900,000 Expected to Switch – and the Facts Many Get Wrong
Published 03.10.2026
According to a Deloitte survey published on 2 October 2026, between 600,000 and 900,000 people are likely to switch basic health insurer for 2027. Almost half would switch for a premium increase of just CHF 20 a month, yet fewer than half answered four basic questions about Swiss health insurance correctly. Our analysis shows that in all 42 premium regions, the cheapest insurer in 2027 is more than CHF 20 a month below the average.
The 2026 Deloitte survey in numbers
Consultancy Deloitte had YouGov survey 1,236 people in German-, French- and Italian-speaking Switzerland in the third quarter of 2026. The backdrop is the 2027 premium increase: according to the federal government, the average premium rises by 5.0% to CHF 412 a month, and for adults by 4.9% to CHF 487.60.
| Survey finding | Value |
|---|---|
| Expected insurer switches for 2027 | 600,000 to 900,000 people |
| Would switch for CHF 20 more per month | almost 50% (previous year: about one third) |
| Review their basic insurance every year | over 50% |
| Have financial worries because of premiums | over 50% |
| Struggle to pay their premiums | about 25% |
| Rate their own knowledge as average or better | 88% |
| Answer four knowledge questions correctly | just under 50% |
According to Deloitte, 18 to 29-year-olds and residents of French-speaking Switzerland and Ticino compare more often than average. About 12.5% already use AI tools for health insurance questions, and around a quarter plan to.
The CHF 20 threshold: what the 2027 premium data shows
We analysed all 1,019 standard-model offers for 2027 (adults, CHF 300 deductible, without accident cover), comparing the cheapest offer with the average of all insurers in the same premium region:
| Cheapest insurer vs. regional average (2027) | Per month | Per year |
|---|---|---|
| Average across all 42 premium regions | CHF 49.60 | CHF 595 |
| Smallest gap (one region) | CHF 29.35 | CHF 352 |
| Largest gap (one region) | CHF 81.50 | CHF 978 |
| Regions with a gap of at least CHF 20 | 42 of 42 | – |
| Regions with a gap of at least CHF 40 | 36 of 42 | – |
So almost everywhere, the gap between the cheapest offer and the regional average is larger than the CHF 20 at which, according to the survey, almost half of people would switch. If you are with an averagely priced insurer today, a comparison will usually show a bigger difference than the premium increase itself.
Four facts about basic insurance that people often get wrong
Deloitte flags the knowledge gap as a risk: according to the survey, people who know the system less well have financial difficulties more often. These rules apply according to the Federal Office of Public Health (FOPH):
- Same benefits: basic insurance (KVG/LAMal) coverage is identical at every insurer by law. Only the premium, deductible and insurance model differ.
- Obligation to accept: every insurer must accept anyone required to be insured in its area into basic insurance, regardless of age or health and with no waiting period. This does not apply to supplementary insurance.
- Deductible: adults choose between CHF 300 and CHF 2,500. With CHF 2,500 the 2027 premium is CHF 1,439 a year lower on average, but you carry up to CHF 2,200 more costs yourself.
- Model: family doctor, HMO and Telmed models cost around CHF 508 a month on average in 2027 with a CHF 300 deductible, versus CHF 568 for the standard model. In return, your first point of contact is restricted.
What you can compare before 30 November
Insurers must send you your 2027 premium by the end of October. Your cancellation of basic insurance must reach your insurer by 30 November 2026; what counts is the date of receipt, not the postmark. The FOPH recommends sending it by registered mail or A-Post Plus by mid-November. The switch takes effect on 1 January 2027. If you have overdue premiums for which you received a reminder, you cannot switch until they are paid.
- Your 2027 premium compared with the cheapest offer in your premium region
- Every deductible from CHF 300 to CHF 2,500, at your insurer and at others
- The standard model versus family doctor, HMO and Telmed models
- Whether you want to keep supplementary insurance: it is separate from basic insurance and can stay with your current insurer
