Swiss Deductible 2027: At What Health Costs CHF 2,500 Pays Off

Choosing a CHF 2,500 deductible instead of CHF 300 makes basic health insurance CHF 1,439 a year cheaper on average in 2027 (adults, standard model, without accident cover). If your health costs stay below roughly CHF 1,900 a year, the high deductible is cheaper overall; above that, the low one is. That is the result of our analysis of all 1,019 standard-model offers for 2027.

How much the deductible saves on your 2027 premium

Besides the ordinary CHF 300 deductible (Franchise), adults can choose CHF 500, 1,000, 1,500, 2,000 or 2,500. According to the Federal Office of Public Health (FOPH), insurers may grant a discount of at most 70% of the additional risk you take on, so no more than CHF 1,540 a year for CHF 2,500. Our analysis of the approved 2027 premiums (adults, standard model, without accident cover, average of all offers):

The break-even shows the annual health costs (bills covered by basic insurance) above which the CHF 300 deductible becomes cheaper overall. Depending on insurer and region, the saving with a CHF 2,500 deductible ranges from CHF 1,294 to CHF 1,527 a year.

The maths: five health-cost scenarios

On top of the deductible you pay a 10% co-payment (Selbstbehalt) on costs above it, capped at CHF 700 a year. The table shows your total annual cost (premium plus deductible plus co-payment) at the average 2027 premium, standard model:

In the worst case, with very high costs, a CHF 2,500 deductible costs about CHF 761 a year more than CHF 300. With low costs you save up to CHF 1,439. Not included is the hospital contribution of CHF 15 per day, which is the same for both deductibles.

New Comparis analysis: deductible, model and insurer combined

According to a Comparis analysis published on 30 September 2026, around 200,000 adults could cut their 2027 premium burden by 40% or more by combining a change of insurer, model and deductible. The largest saving it reports is CHF 3,498 a year. Even people who already have the CHF 2,500 deductible could save up to 29% in some regions by switching insurer, according to Comparis.

In our data, a Telmed offer with a CHF 2,500 deductible costs CHF 390.85 a month on average, compared with CHF 568.10 for the standard model with CHF 300. That is a difference of around CHF 2,127 a year (average of all 2027 offers). Basic insurance (KVG) coverage is identical at every insurer by law.

What you can compare before 30 November

Your insurer must send you your 2027 premium by 31 October at the latest. According to the federal government, you should tell your insurer about a deductible change by 15 November. To switch insurer, your cancellation must reach your current insurer by 30 November 2026; the switch takes effect on 1 January 2027.
