Premium Regions 2027: How Much Your Swiss Address Changes Your Health Insurance Premium

Your Swiss basic health insurance premium depends on which premium region your municipality belongs to. 11 cantons are split into two or three regions. Our analysis of 2027 premiums shows adults in canton Zurich pay on average about CHF 99 more per month in region 1 than in region 3, close to CHF 1,200 a year. Within the same region, however, the gap between the cheapest and most expensive insurer is often even larger.

What a premium region is

According to the Federal Office of Public Health (FOPH/BAG), your basic insurance premium depends partly on the premium region of your municipality of residence. Cantons with significant local differences in healthcare costs can be split into two or three regions, based on costs per insured person. The Federal Department of Home Affairs sets the maximum permitted premium differences between regions for each canton.

An updated allocation of municipalities applies from 1 January 2027; the FOPH has published the list of premium regions valid from that date. Basic insurance (KVG/LAMal) benefits are identical in every region and at every insurer.

Within your region, the spread is often bigger

Few people move house to save on health insurance. But within the same premium region, the 2027 figures show gaps that often exceed the regional effect (same basic cover, adults, standard model, CHF 300 deductible, without accident cover):

Across all 42 premium regions, the cheapest insurer in 2027 is on average CHF 49.58 per month below the regional average, close to CHF 595 a year. The largest spread in a single region is CHF 240.10 per month.

Moving house: your premium follows your new address

Because the premium is tied to your municipality of residence, it changes when you move to a different premium region or canton. You need to report your change of address to your insurer, which then adjusts the premium to the new region. Moving within the same region does not change your premium.

What you can compare before 30 November

Your insurer must tell you your 2027 premium by 31 October at the latest. Notice to cancel basic insurance must reach your insurer by Monday, 30 November 2026; the switch takes effect on 1 January 2027.

How many premium regions are there in Switzerland?

There are 42 premium regions. 11 cantons are split: Bern, Graubünden, Lucerne, St. Gallen and Zurich into three regions; Basel-Landschaft, Fribourg, Schaffhausen, Ticino, Vaud and Valais into two. The other 15 cantons are one region each.

How big is the premium gap between region 1 and region 3?

In 2027, adults (standard model, CHF 300 deductible, without accident cover) in canton Zurich pay on average CHF 98.61 more per month in region 1 than in region 3; in canton Bern it is CHF 101.73. That is about CHF 1,200 a year.

How do I know which premium region I live in?

It depends on your municipality of residence. The FOPH publishes the allocation of every municipality; a premium comparison using your postcode applies the region automatically.

Is the cover the same in every premium region?

Yes. By law, basic insurance benefits are identical at every insurer and in every region. Only the price differs.
