Switching insurer, lowering your deductible, or changing your model can all cut your premium — but there's a fourth lever that doesn't depend on shopping around at all: Prämienverbilligung, the income-based premium subsidy every Swiss canton is legally required to offer. It's paid on top of whatever plan you choose, and many eligible households never claim it.
What is Prämienverbilligung?
Under Article 65-66a of the Federal Health Insurance Act (KVG) and its implementing ordinance (VPVK), every canton must reduce basic-insurance (KVG/LAMal) premiums for residents in modest economic circumstances. It's a direct subsidy toward your premium bill — it doesn't change your coverage, which stays identical everywhere by law, and it doesn't restrict which insurer, deductible, or model you pick.
Who qualifies
Eligibility is decided by your canton based on taxable income and assets, and thresholds differ significantly canton to canton — there's no single Swiss-wide cutoff. Federal law sets minimum guarantees cantons must meet: children from lower- and middle-income families must receive at least an 80% premium reduction, and young adults in education or training at least 50%. Nationally the federal government covers 7.5% of total basic-insurance costs toward these subsidies (distributed to cantons by population), and cantons add their own funding on top; since 1 January 2026 cantons must also meet a new minimum contribution tied to the premium burden on their lowest-income residents.
Automatic or do you have to apply?
This is where cantons diverge most. Some assess eligibility automatically from your tax return and pay the subsidy directly to your insurer — no form needed. Others require an application, usually annually, with your canton's Sozialversicherungsanstalt or Ausgleichskasse (social insurance office). If you already receive federal supplementary benefits (Ergänzungsleistungen) or certain social assistance, you typically get the subsidy without a separate application. Deadlines also vary by canton and can change year to year.
How to check your canton's rules
Because amounts, thresholds and process are set canton by canton — and change regularly — start with your canton's Sozialversicherungsanstalt or cantonal tax office, or the Federal Office of Public Health's cantonal contact list via priminfo.admin.ch / bag.admin.ch. If you moved cantons or your income changed this year, it's worth checking again even if you didn't qualify before.

Prämienverbilligung is independent of, and paid on top of, whichever insurer, deductible, and model you choose. A lower base premium from comparing insurers can mean a bigger subsidy in francs too, since most cantons calculate the reduction as a share of your premium — so comparing before 30 November still pays off even if you already receive a subsidy.
